Patient Service Questionnaire

To continuously improve the quality of care received by our patients, we conduct an ongoing review of the service provided. To help us identify problem areas and/or concerns, we have developed a questionnaire survey for you to complete. Please answer all questions and document any concerns in the space provided below.

1. When you were discharged from Surgery Center of Weston, did you receive adequate information regarding your discharge instructions?
2. When you have contact with the facility’s staff, are you treated in a courteous manner?
3. Was the staff (ie nurse and /or physician) under your care, dependable to your needs and cordial?
4. Are you given clear instructions and education regarding your procedure?
5. Do you feel that the facility staff (nurse and or physician) sanitized their hands before procedure?
6. Did the Nurse or Physician question you as to the level of your pain?
7. Were you treated in a respectful and supportive manner by our staff during your visit(s)?
8. Were mutual goals of your procedure discussed at the tiome of admission with your physician?
9. Was your overall wait period before your procedure less than 60 minutes?
